Case Management Care Coordination Organizations---New York State
As of July 1, 2018, the system of Case Management in New York State has been changed. Case Management is no longer part of the many OPWDD based non-profit agencies that have served our communities. All Case Mangers are now Health Home Care Coordinators employed by one of seven Care Coordination Organizations formed across the state. The case managers work to insure that clients get appropriate services from the local OPWDD based agencies, as well as medical, mental health, and social services.
These are the new Care Coordination Organizations. You can contact a local OPWDD agency for assistance in registering for services.
